I accept that Mons Calpe SC "the Club" holds my personal data/my child’s personal data and I hereby consent to the processing by the Club of my personal data/my child’s personal data for any purpose related to the registration process on the Comet or TMS Platforms, as required.

I, the legal parent/guardian of the above-mentioned player give my consent for my child to play as a feeder player into the an older age Group of the Youth Leagues organised by the Gibraltar FA or in any friendly or tournament organised by the Club outside Gibraltar.

On registering the Child, I, the legal parent/guardian of the above-mentioned player accept my obligation and those of my child to observe, abide and adhere to the Code of Conduct for players and parents.

(Only for Children aged 16 or 17 or for a child who will attain the age of 16 from the date this document is signed to 30th April 2025). I, the legal parent/guardian of the above-mentioned player give my consent for my child to play in any of the competitions organised by the Gibraltar Football Association in the Intermediate and Senior National Competitions and/or any friendly game or tournament organised by the Mons Calpe SC outside Gibraltar.
